Proudmouth Club

Proudmouth Club membership is valid for
12 months from the date of purchase.

• The Proudmouth Club is not transferable and cannot be redeemed for cash. However, we are able to offer annual membership as a gift, packaged for another individual.

• Membership includes the following:
  • Two hygienist treatments with optional "bitewing" x-rays
  • One examination by a dentist
  • Panoramic x-ray
  • Maintenance pack
• Membership cannot be 'put on hold' or deferred. The 12 month term is an integral part of promoting good oral hygiene through regular care.
• The products that are provided are specifically chosen for their home care suitability, however Proudmouth Dentistry reserves the right to substitute any makes or models shown in any advertising material with other products of equal or higher standards.
• Treatments are available at any Proudmouth Dentistry practice.
• The Proudmouth Club membership is designed to support good oral hygiene. It is not a guarantee that you will not require further dental treatment.
• Proudmouth Club memberships cannot be used in conjunction with any other offer, apart from discounts offered in the terms and conditions. No other discounts apply.
• Appointment dates made upon registration are provisional.
• Patients may change the date of their appointment; however it is their responsibility to complete the treatments before the expiry date of their membership. You must notify us with at least 48 hours notice if you are unable to attend an appointment.
• If any provider (dentist or hygienist) who was originally booked is not available at the appointment date, the treatment will be with another suitably qualified provider.
• By joining the Proudmouth Club and receiving the discounted, packaged prices, the member accepts and agrees to be bound by these conditions of membership.
